I want to add that having spent decades in health care, and having implemented such a culture in three facilities, these management tactics are discouraged, and often they are extinguished, and the perpetrators punished. 

If Health Care could adopt a less shaming approach internally, I think it would mean those in that industry shift their thinking from blame and shame  (lawsuits ‘n’ liability) to something far more healing: a culture of safety.
